I just printed off one that said approved in April of 2014. Does anyone know if this is the correct form, or do we need another? I did not notice this until after sending a kid to get checked out for poison ivy. The kid was almost denied to compete today because of that. It would be nice if that date was placed somewhere clearly visible in the middle of the paper so there is no future problems with printers.
